Initiates health records for inmates. Maintains confidentiality of all medical record information. Monitors and maintains accountability for all health records on file.

Maintains health records in accordance with established policies and procedures.

Pulls and prepares health records for practitioner (medical, dental and mental health) sick call, chronic care clinics and transfers.

Maintains all active health records in alphabetical order.

Forwards all diagnostic test results to the appropriate practitioner for review and countersignature. Complies with the rules, directives, policies, procedures and protocols of the company. Orders and maintains appropriate levels of health services forms and clerical supplies.

Reviews records for completeness. Participates in Quality Improvement activities.

Files loose paper work and performs other clerical duties as assigned

Communicates to supervisors all difficulties in providing records upon request. Retrieves records from clinic periodically throughout shift.

Integrates records from previous incarcerations, Infirmary and Mental Health records in current records when applicable. Researches intakes from previous shift. Copies and mails records in response to authorized releases/requests.

Pulls all records and corresponding labs for inmate worker medical clearances; files copy of clearance in record. Pulls all records for Administrative review for corresponding grievances; files copy of grievance in record. Other duties as assigned.

It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management.

Ability to maintain accurate documentation and records is required.

Must have knowledge of common safety precautions.

Must be able to perform tasks as outlined in the facility's Procedure Manual and in accordance with all applicable State and Federal regulations.

Education and Experience:

High school diploma or GED required.

1-2 years' experience is desirable, preferably in a correctional or medical setting.

C.N.A., E.M.T., C.M.A., or Medical Assistant preferred.
